How in-home care works

No waiting room. No commute.

Marland travels to you across New York City, Long Island and Westchester. Every appointment — consultation, bloodwork, infusion, injectable — takes place in your own home, at a time that suits you, with the supplies and setup brought in and taken away again.

Goals, history and testing

The first consultation can be virtual or in your home. He reviews your history and goals, and orders laboratory testing where it is clinically appropriate. Nothing is administered on a first visit until the panel is back.

Care in your own space

He arrives with everything required — sterile supplies, portable equipment, sharps disposal — sets up, treats, and clears the space afterwards. Most appointments run 30 to 90 minutes.

Follow-up and adjustment

Plans are reviewed against how you respond and adjusted over time. Follow-up is virtual or in person, and aftercare questions go directly to Marland.

What to expect. Care is provided by Marland Kaplan, NP personally, or by a licensed clinician employed directly by Priorityou. Services are offered throughout New York City, Long Island and Westchester; your address is confirmed privately after scheduling. A consultation is required before any treatment, and not every consultation results in a prescription or procedure — some conclude with testing, or with advice and no treatment at all. Treatment recommendations are individualized and depend on medical appropriateness.

Weight management here is a clinical program, not a prescription pad. Labs first, then GLP-1 therapy where it is appropriate, alongside protein targets, resistance training and monthly review. Weigh-ins happen at home, with your clinician present, and the plan moves with the numbers.

Where you're starting from

BMI is a blunt instrument. It cannot tell muscle from fat, it reads differently across body types and ancestries, and an athlete and a sedentary person can share the same number. What it is good for is a starting point — something to put on the table on day one, next to your labs, your history and how you actually feel.

This is not a diagnosis and not an eligibility test. Nothing here determines whether any treatment is appropriate for you. Candidacy for weight-management therapy is decided in consultation, after evaluation and bloodwork, by a licensed clinician. Nothing you enter is sent anywhere or stored — the arithmetic happens in your browser.

What we look at instead

  • Waist-to-height ratio. Keep your waist under half your height. Tracks metabolic risk better than BMI and needs only a tape measure.
  • Body composition. Muscle and fat weigh the same on a scale and behave nothing alike in the body.
  • Metabolic panel. Fasting glucose, HbA1c, lipids, liver enzymes, thyroid. Where the answers usually are.
  • Trend, not snapshot. One number on one morning tells you almost nothing. The direction over months tells you most of it.

Meet Marland

Marland Kaplan, NP

Marland Kaplan started in intensive care. ICU nursing teaches a particular kind of attention: you learn to read a patient before the monitor tells you anything, and you learn that the distance between stable and not stable is usually a detail somebody caught early. He carried that into practice as a nurse practitioner, first in internal medicine and then in urology, where the work is long-term and rarely dramatic. Chronic disease. Medication lists that interact. Lab trends followed over years. Conversations most people would rather not have. He has practiced functional medicine extensively alongside all of it, which is where the habit of reading a whole panel comes from rather than chasing the one value the lab flagged.

He came to injectables and regenerative medicine already fluent in anatomy, dosing and risk, and already used to patients who turn out to be more complicated than they first appear. He asks about your medications before he asks about your goals. He orders labs before he writes a protocol. When a treatment is not appropriate for you, he says so and explains why.
